Terms of Service 

Effective Date: January 1, 2021

Please note, entering into a membership agreement with the practice is not an insurance plan and is not intended to replace or substitute any health insurance plan or health insurance coverage that you may carry. We encourage you to purchase comprehensive insurance from a health insurance company. The membership fee does not affect the co-payments, co-insurance or deductibles that you are required to pay pursuant to the terms of your insurance coverage. You will continue to be financially responsible for any co-payments, coinsurance or deductible amounts required by your insurer.

Upon joining the practice, patients will enter into a Membership Agreement with the practice, which can be terminated at any time by either party.

The monthly membership fee covers services not reimbursable by insurance and/or Medicare, such as program materials, unlimited email support with your provider, and their cell phone for call/text emergencies.
